Louisville, KY : Woman killed in head-on collision with truck near Highland Park early Monday morning, August 15, 2016

A woman is dead following a head-on collision between a minivan and a box truck early Monday morning near Highland Park.

According to Louisville Metro Police spokeswoman Alicia Smiley, officers from the department's sixth division and traffic unit responded to the crash near Park Boulevard and E. Woodlawn Avenue around 4:30 a.m.

Gladys Hernandez, 29, the driver of the minivan, died at the scene, Smiley said. The male driver of the truck was taken to University of Louisville Hospital with what appeared to be non-life-threatening injuries.

The cause of the crash was still under investigation, though Hernandez was wearing a seatbelt at the time of the collision, according to deputy coroner Larry Carroll.
