Troopers say Michael Jennette, 41, of East Palatka, was driving his 1995 Honda Accord west on U.S. 17 at North 9th Street at 5:25 p.m. and David Barker Jr., 33, was driving a 2000 Honda Accord east on U.S. 17 with Guay and four juveniles inside. Both vehicles entered the intersection and collided, with the front end of Jennette's car hitting the left side of Barker's car.
Troopers say Jennette's car spun counterclockwise before coming to rest on Reid street in the right lane. After the impact, Barker's car hit a large concrete traffic light pole.
According to the FHP report, Guay was not wearing a seat belt and died at the scene.
Jennette was wearing a seat belt and was taken to Putnam Community Medical Center with minor injuries. Barker was also wearing a seat belt and was taken to Putnam Community Medical Center, then transferred to UF Health Jacksonville with serious injuries.
Troopers did not release any information regarding the names and conditions of the four juvenile passengers in the car because of juvenile confidentiality law.
Investigators have not determined if alcohol was a factor in the crash.
