New Carlisle, OH : Driver killed in a fatal crash on Route 40 at Lake Road on Wednesday, Sep 9, 2015

The Ohio State Highway Patrol is investigating a fatal crash.

Investigators say a pickup truck rear ended a car along National Road at North Lake Road in New Carlisle. The pickup then veered into the other lane, hitting an oncoming car head-on. That driver was killed. Another driver was removed to Miami Valley Hospital for treatment. The driver of the pickup was not hurt.

Exactly what caused the crash is under investigation.
