Gainesville, TX : A 59-year-old Double Oak woman killed in a head-on collision on Monday, June 26, 2017

Authorities have identified a 59-year-old Double Oak woman killed in a head-on collision near Valley View.

The woman’s husband, a passenger in the vehicle, did not have any reported injuries, while a 26-year-old Valley View man driving another vehicle involved in the crash remained in a Denton hospital Monday.

Staff Sgt. Mark Tackett, a spokesman for the Texas Department of Public Safety, said Bonnie Slack was pronounced dead at the scene by Precinct 1 Justice of the Peace Olivia Neu.

Tackett said troopers were dispatched to the wreck about three and a half miles east of Valley View at 6:51 p.m. on Thursday.

Steven Ingram was driving a 2007 Ford Sport Trac eastbound on Farm-to-Market Road 3002 when the vehicle crossed over the center line and collided head-on with a 2014 Chevy Silverado pickup traveling westbound, he said.

Bonnie Slack, who was driving the Chevy truck, was traveling with her husband, David Slack, according to Kevin Grant, director of Cooke County Emergency Medical Services.

Grant said David Slack refused to be taken to an area hospital. David Slack did not have any reported injuries, according to law enforcement officials.

A Cooke County EMS ambulance took Ingram to Medical City Denton. His condition and what type of injuries he suffered were not released.

Tackett said the crash remains under investigation.
