Salt Lake City, UT : 4-year-old died and three people injured after Taylorsville vehicle crash on Saturday, Nov 7, 2015

Police say two children were critically hurt and two adults were also injured after a head-on collision in Taylorsville.

Unified police Lt. Lex Bell says a 4-year-old and a 3-year-old were in extremely critical condition Sunday.

Bell says police initially reported the 4-year-old had died but the child was revived at the scene and airlifted to a hospital.

According to Bell, the accident happened Saturday around 3:15 p.m. when the car driven by the children's mother crossed the median and collided with a truck.

The 31-year-old mother is in serious condition. The 72-year-old man driving the truck is in fair condition.

Bell says the 4-year-old was sitting in the front seat and neither child was properly restrained.

Police are looking into whether impairment or distracted driving was a contributing factor.
