BURLINGTON, IA : 1 dead after forklift accident on Tuesday, 21st May 2024

A man is dead after a forklift accident, according to officers with the Burlington Police Department.

Sam K. DeJanes, 64, was identified as the man who died as a result of the forklift accident, according to a media release from BPD. The incident remains under investigation.

Tuesday at approximately 1:46 p.m., officers with BPD were called to assist the Burlington Fire Department with a reported forklift accident at Industrial Service Corporation, 1731 N. Roosevelt Ave., according to a media release from BPD. Upon arrival, the victim was found and initially taken to Southeast Iowa Regional Medical Center by ambulance.

Later, the victim was taken to University of Iowa Hospital where he succumbed to his injuries.
