Eddyville, OR : 35-year-old Andrew Koloski dead and two others injured in a two-vehicle crash on Highway 20 in Lincoln County on Monday morning, April 6, 2015

According to Lieutenant Justin McGladrey, on April 6, 2015, at approximately 12:07 a.m., a 2000 Dodge Durango sport utility vehicle, driven by Ivan Sandria Cruz, 24, of Newport was westbound on Highway 20 near milepost 28. A 1995 Honda Accord station wagon being driven by Andrew Raymond Koloski, 35, of Springfield, was eastbound at the same location.

The cause of the crash is still under investigation. However, police say it is known that the vehicles collided nearly head-on. Koloski was pronounced deceased at the scene.

Sandria Cruz and a 7-year-old male passenger were transported by ground ambulance to Good Samaritan Hospital in Corvallis where Cruz is receiving treatment for non-life threatening injures. The juvenile passenger was treated and released from the hospital.

Highway 20 was blocked for most of the morning as investigators processed the crash scene.

OSP was assisted at the scene by members of the Lincoln County Sheriff's Office, Toledo Fire Department, ODOT, Pac West Ambulance, Lincoln County District Attorney's Office, and Lincoln County Medical Examiner.
