The truck driver was not injured.
Rescue workers pulled Robert Piland, 45, from the wreckage of his Chevy Tahoe and took him to the hospital.
Troopers later cited Piland for Driving Under the Influence of Intoxicants.
The crash happened around 8:17 p.m. in the Medford area.
Piland was northbound in his Tahoe when the SUV traveled over the center median and into the southbound lanes, state police said.
