San Juan County, UT : 6 people killed, 1 critical after head-on collision in San Juan County Sunday on July 6, 2014

The death toll from Sunday’s head-on crash on Highway 191 in San Juan County rose to six on Monday, while a seventh person injured in the accident remained in very critical condition.

Utah Highway Patrol Sgt. Todd Royce said the latest fatality was the fourth person to have died from a northbound van that veered into the path of southbound sports, apparently while trying to pass another vehicle.

Both occupants of the car also died in the crash near mile post 81, about 10 miles north of Monticello, about 3:22 p.m. Sunday.

The van occupants who died were: the driver, Samantha Blueeyes, 23; Alfreda A. Bowman, 28; Michael A. Blueeyes, 22; and Esmerelda Valesquez, 11. The sole survivor from the van was Travis L. Howland, 24. All of the van occupants were from Salt Lake City, according to the UHP.

The two who died in the sports car were: the driver, Cody R. Farrabee, 22, of Queen Creek, Ariz.; and Rheana Hazel, 23, of Las Vegas, Nev.

In Tooele County on Saturday, a woman and her two daughters died on Interstate 80, about 30 miles east of Wendover, after their Chevy Suburban crashed head-on into a wrong-way pickup truck, according to the UHP.
