Phoenix, AZ : RV driver died in rollover crash on Loop 101 southbound at the Thomas Road off ramp on Thursday morning, Oct 22, 2015

The Arizona Department of Public Safety has released the name of the RV driver who died Thursday morning in a rollover crash.

The victim is Brian A, Smith, 56, of Surprise, according to DPS spokesman Quentin Mehr.

Troopers said that driver's wife witnessed the accident as she drove behind the RV in a separate vehicle along Loop 101 southbound at the Thomas Road off ramp shortly after 9 a.m.

Smith reportedly lost control of the RV which veered off the road and flipped over.

Smith was pinned under the RV and was declared dead at the scene.

The driver's wife was one of the reporting parties. She had been traveling behind the RV in a separate car and observed erratic driving behavior before the RV rolled over.
